This form allows any member of the public to submit accessibility requests related to a wide variety of issues, including curb ramps, sidewalk obstructions and access to buildings, facilities and programs. The form will be reviewed by the City of Kent Americans with Disabilities Act (ADA) Coordinator and a response will be provided via email. Alternate forms of communication will be made available upon request. (To report a situation that may endanger pedestrians or bicyclists, please call Public Works Operations at 253-856-5600).

Grievance Procedure under the ADA

Additionally, the City has an adopted Grievance Procedure in place, established to meet the requirements of Title II of the ADA. This procedure may be used to file a formal complaint alleging discrimination on the basis of disability in the provision of City services, activities, programs, and facilities.

Within 15 calendar days after receipt of the complaint, the ADA Coordinator or his/her designee, will meet with the complainant to discuss the complaint and the possible resolutions.
